Case Management

Case management is at the heart of the work we support. Families meet one-on-one with a trusted, compassionate professional who helps guide them out of crisis and toward stability. Case managers work intensively with families for up to 18 months, creating personalized plans to help them stabilize and thrive. The Siemer Institute provides challenge grants to fund case workers, ensuring families receive the dedicated support they need.

Our Process

The Siemer Institute provides challenge grants to fund case managers or coaches who support our partners in helping families achieve stability. Our partners focus on establishing housing stability for each family to prevent unplanned moves. Once the immediate crisis is addressed, families work with case managers to set goals, develop financial management skills, and explore ways to increase their income. Many families learn budgeting techniques to maximize their resources, while others save for emergencies or pursue education, such as obtaining a GED or technical training, to enhance their earning potential. Our partners take a holistic 2Gen approach, addressing the needs of both parents and children, empowering families to set and achieve meaningful goals with the guidance of dedicated case managers and other staff.
